Client Information Update Form
A move or a new phone number is easier to record when the client signs the change. This client information update form asks what changed, the new address, phone, or email, the effective date, and a signature. It collects contact details only. What is a client information update form?
A client information update form is the signed note of a contact change: what changed, the new details, and when they take effect.
When to use it
- When a client moves or changes a phone number.
- When the person you should email is no longer the right contact.
- When the business name on your records needs an update.
- When you want a signature on the change itself.

Why FormSign
Each update becomes a row on your FormSign board. The starter letter uses each question's label as a placeholder, so a copy of the change matches the form.
This template is a starting point, not legal advice. It records contact details only.
Questions
Which changes can a client report?
Address, phone, email, contact person, or business name. They check every one that applies.
Should they enter a payment account?
No. This form has nowhere for that. It records contact details only.
What if only the phone number changed?
They check Phone, fill in the new phone, and leave the other new fields blank.
Can I add a field we already track?
Yes. Add it in the builder if your records include something this list skips.
Do I need an account to try it?
No. Open the template, edit the checklist and test it. You'll create an account when you want to save it and send it.
