Accounting New Client Intake Form
Before you quote a new accounting client, you need to know what kind of entity they are, what they need, how their books look today and what deadlines are close. This intake form asks those questions up front, so your first call is about the work, not about collecting basics. Edit it for your services, try it, then put the link on your website or send it to every new inquiry.

What is an accounting new client intake form?
It's the questionnaire a prospective client fills in before an accounting or tax firm takes them on. It covers who they are, what entity type they have, which services they need, how their records look and whether anything is overdue, so the firm can decide fit and quote accurately.
When to use it
- On your website's "work with us" page.
- When a referral comes in.
- Before a consultation call.
- When several people in the firm take inquiries and should ask the same questions.
What to include
Contact details
Name, email, phone and business name.
Entity type
It changes the returns and the price.
Services needed
A checklist of your services.
State of the books
Software, spreadsheets or nothing yet.
Overdue returns and deadlines
Know about urgent work before the first call.
Who decides
Saves a follow-up.
What to leave out
Don't collect Social Security numbers, bank details or tax documents at the inquiry stage. Ask for those after the client signs on, through a secure process.
Why FormSign
Every inquiry becomes a row on your board, so you can sort new leads by entity type, services or deadline. When a client says yes, the same workflow can send your engagement letter, filled from their answers, for e-signature.
Questions
How is this different from a tax organizer?
The intake form screens a new client before you take them on. A tax organizer collects year-end details from a client you already serve.
Should I ask for tax documents here?
Not yet. Ask after the client signs on.
Can I turn the intake into an engagement letter?
Yes. Add your letter as a document in the same workflow and FormSign fills it from the answers and sends it for e-signature.
Can I list my own services?
Yes. Edit the options in "What do you need help with?".
Do I need an account to try it?
No. Open it, edit it and test it. You'll create an account when you want to save and share it.
