Employee Check-In Form
A good weekly check-in takes two minutes to answer and tells you what a one-on-one would have missed: who is stretched, what is stuck, and who needs a hand. This employee check-in form asks a few honest questions each Friday. Change the wording, try it yourself, then share one link with your team every week.

What is an employee check-in form?
An employee check-in form is a short, regular questionnaire that asks people how their week went. Unlike a yearly survey, it's quick and frequent, so small problems such as an overloaded teammate or a stuck project surface while they're still easy to fix.
When to use it
- Every Friday (or every other week) for teams of any size.
- For remote or hybrid teams where you don't see people every day.
- Before weekly one-on-ones, so the conversation starts with what matters.
- During a busy period, launch or change, when workload can shift fast.
What to include
A simple overall rating
One 1-5 question about the week gives you a quick read over time.
Workload
Ask directly whether it's too light, about right or too heavy.
Wins
People share what went well when you ask.
Blockers
An open question about what got in the way.
A clear ask for help
Make it easy to say "I need help" and how soon.
Next week's focus
Keeps priorities visible.
Honesty about privacy
Say who reads the answers. If names are optional, say that too, and don't promise anonymity a manager can see through.
Why FormSign
Publish the check-in once and share the same link each week. Every response lands as a new row on your FormSign board, so you can sort and filter by week, team or workload and see the history build up without a spreadsheet. You can also switch it to a one-question-at-a-time conversational layout if that suits your team better.
Questions
How many questions should a weekly check-in have?
Keep it short enough to finish in about two minutes. This template has three required rating questions and a handful of optional open questions; remove anything your team won't use.
Should a check-in be anonymous?
It's your choice, but be honest about it. In this template the name is optional, and the form says your manager reads every answer. If a team is small, answers can be recognizable even without a name, so don't promise full anonymity.
How often should employees fill it in?
Weekly works for most teams. If people start skipping it, try every two weeks before you drop it.
Can I send the same form every week?
Yes. Publish it once and share the same link each week. Each response is a new row on your board, with the week-ending date so you can compare weeks.
Can I see trends over time?
You can sort and filter responses on the board by week, team or answer. FormSign doesn't draw charts for check-ins yet.
Do I need an account to try it?
No. Open the template, edit it and test it. You'll create an account when you want to save it and share it with your team.
