Job Application Form

Most small businesses don't need an applicant tracking system; they need one clean application that every candidate fills in the same way. This job application form covers contact details, the position, availability, work history, references and a signed certification. Edit it for your roles, try it, then share the link wherever you post the job.

What is a job application form?

A job application form (or employment application) is a standard set of questions every candidate answers, so you can compare people fairly. It usually covers contact details, the role, availability, work history, references and a signed statement that the answers are true.

When to use it

  • When you post a job and want every applicant to give you the same information.
  • For hourly and frontline roles where resumes vary or aren't common.
  • When you want a signed certification on file for each applicant.
  • When several managers review applications and need one shared view.

What to include

Contact details

Name, email, phone.

The role

Position, full- or part-time, earliest start date and availability.

Eligibility

Work authorization and sponsorship questions, asked the same way for everyone.

Experience

Most recent job, a summary, and an optional resume upload.

References

Two is usually enough.

Certification and signature

A statement that the answers are true, signed and dated.

What to leave out

Avoid questions about age or date of birth, race, religion, national origin, marital or family status, disability, or pregnancy. Many states and cities also limit questions about salary history and criminal records on applications. Check the rules where you hire.

Why FormSign

Every application lands as a row on your FormSign board with one column per question, so you can sort by role, start date or availability and share the board with the people helping you hire. When you make an offer, the same workflow can carry the candidate into your onboarding form and offer letter, which FormSign fills from their answers and sends for e-signature.

This template is a starting point, not legal advice. Hiring rules vary by state and city; review the questions with your HR advisor before you use them.

Questions

What questions shouldn't be on a job application?

Avoid questions about protected characteristics such as age, race, religion, national origin, sex, marital status, disability or pregnancy. Many places also restrict questions about salary history or criminal records at the application stage. Check the rules where you hire.

Can applicants sign the application online?

Yes. The template ends with a certification checkbox and a signature question.

Can applicants attach a resume?

Yes. There's an optional file upload. Keep it optional if you hire for roles where resumes aren't common.

How do I review applications?

Each application becomes a row on your FormSign board. Sort and filter by position, availability or start date, and share the board with your hiring team.

Can I use one form for several open roles?

Yes. Edit the "Position" options to list your open roles, then filter the board by position.

Do I need an account to try it?

No. Open the template, edit it and test it. You'll create an account when you want to save it and share the link.
