Mileage Log Form
A trip is easier to look up later when the places, the purpose, and the miles are in one row. This mileage log form asks for the driver, the date, the start and end, the purpose, both odometer readings, the miles, the vehicle, any receipts, and notes. It does not set a rate, and it does not say how the miles should be treated. Edit the vehicle list, try it, then use one link for each trip.

Why FormSign
Each trip becomes a row on your FormSign board. The starter letter uses each question's label as a placeholder, so a copy of the log matches the form.
This template is a starting point, not legal or tax advice. It records a trip. It does not set a rate or say how the miles should be treated.
Questions
Does this form set a mileage rate?
No. It records the trip and the miles. Any rate you use stays outside this form.
Does it say whether the miles qualify for anything?
No. It is a trip record only. How you use the miles stays in your usual process.
What if they do not know the odometer?
They can still enter the miles. Leave the odometer fields blank if they were not written down.
Can I change the vehicles?
Yes. Edit the dropdown so it matches the vehicles you actually use.
Do I need an account to try it?
No. Open the template, edit the vehicles and test it. You'll create an account when you want to save it and send it.
