Client Feedback Survey
A finished project is a good time to ask what worked. This client feedback survey asks for an overall rating, how likely they are to recommend you, what went well, what could be better, and whether you may contact them. Edit the wording, try it, then send one link when the work is done.

What is a client feedback survey?
A client feedback survey is a short form you send after the work. It asks for a rating, a recommendation score, two written notes, and permission to follow up.
When to use it
- When a project or a month of work has just finished.
- When you want the same five questions for every client.
- When comments should land on a board, not in a personal inbox.
- When you only want to follow up with people who said you may.
What to include
A rating
One overall score from 1 to 5.
A recommendation score
How likely they are to recommend you, from 1 to 5.
Two written notes
What went well, and what could be better.
Permission to follow up
A checkbox they can leave unchecked.
Why FormSign
Each response becomes a row on your FormSign board, so you can read the comments in one place. The starter letter uses each question's label as a placeholder if you want a short summary document later.
This template is a starting point, not legal advice. Contact a client about their answers only if they said you may.
Questions
Will everyone be asked to talk to us?
No. Follow up only when they check that it is OK to contact them.
What do the numbers mean?
1 is the low end and 5 is the high end, both for the rating and for how likely they are to recommend you.
Can I add the client's name?
Yes. Add a name question if you are not already sending each person their own link.
Can I change the questions?
Yes. Keep it short. Two scores and two comments are enough for most teams.
Do I need an account to try it?
No. Open the template, edit the questions and test it. You'll create an account when you want to save it and send it.
