Client Referral Form
An introduction is easier to follow when the names and the service are written down. This client referral form asks who is referring, who is being referred, which service they need, and whether you may mention the referrer's name. It does not promise a reward or a fee. Edit the services, try it, then send the link to people who offer introductions.

What is a client referral form?
A client referral form is a short introduction. It names the person making the introduction, the person they have in mind, and the service to discuss. Nothing is promised in return.
When to use it
- When a current client wants to introduce someone.
- When you want the same questions for every introduction.
- When you need permission before you mention who sent them.
- When introductions should land on a board, not in a personal inbox.

Why FormSign
Each introduction becomes a row on your FormSign board. The starter letter uses each question's label as a placeholder, so a note to the team matches the form.
This template is a starting point, not legal advice. It only introduces a person. It does not promise anything in return.
Questions
Does this form promise a reward or a fee?
No. It only collects the introduction. Do not add a promise of payment or a gift.
Will you tell the new person who referred them?
Only if the referrer says you may mention their name. Otherwise, ask first or leave the name out.
Which services are on the list?
Bookkeeping, tax prep, payroll, and other. Edit the choices so they match what you offer.
Can I remove the phone number?
Yes. Delete that question if email is enough.
Do I need an account to try it?
No. Open the template, edit the services and test it. You'll create an account when you want to save it and send it.
