Employee Availability Form
A schedule is easier to draft when you already know the days someone can work. This employee availability form asks for their name, email, the period, the days they are available, preferred hours, a maximum number of hours in a week, dates they cannot work, and notes. It does not set the schedule. Edit the hour choices, try it, then send one link before you build the next period.

Why FormSign
Each reply becomes a row on your FormSign board, so you can sort by the period. The starter letter uses each question's label as a placeholder, so a copy of the reply matches the form.
This template is a starting point, not legal advice. It records when someone can work. It does not set a schedule by itself.
Questions
Does this form set the schedule?
No. It records when they can work. You still publish the schedule the way you already do.
What if they can work every day?
They check every day that applies, from Monday through Sunday.
What belongs in dates unavailable?
The specific dates they cannot work in this period, such as a trip or an appointment.
Can I change the hour choices?
Yes. Edit the dropdown so it matches the parts of the day you actually schedule.
Do I need an account to try it?
No. Open the template, edit the hour choices and test it. You'll create an account when you want to save it and send it.
