Employee Handbook Acknowledgment
A handbook only helps if you can show the employee received it. This employee handbook acknowledgment asks for their name, the version they were given, and a signature. Change the wording to match your handbook, try it yourself, then send one link to each new hire.

What is an employee handbook acknowledgment?
An employee handbook acknowledgment is a short signed form that records who received the handbook, which version it was, and that they will read it. It does not replace the handbook itself.
When to use it
- On or before a new hire's first day, with the handbook.
- When you publish a new handbook version and need a fresh signature.
- When several people start in the same week and you want the same record for each.
- When you want the signed acknowledgment on the same board as the rest of onboarding.
What to include
Who is signing
Name, email, job title and start date.
Which handbook
A version name or the date they received it.
A clear statement
They received the handbook and will read the policies in it.
A signature and date
So you have a record, not just an email.
What to leave out
Don't ask for a Social Security number, bank details or routing numbers on this form.
Why FormSign
Each signed acknowledgment becomes a row on your FormSign board, so you can see who has finished and who has not. Add your handbook file or a welcome letter to the same workflow and FormSign can send it with the form. The starter letter uses each question's label as a placeholder, so the signed copy matches the form.
This template is a starting point, not legal advice. Handbook rules vary by state; check what applies to your business.
Questions
Does signing this mean they have read the whole handbook?
Only if your wording says that. This template says they received it and will read it. Change the sentence if you need a different promise.
Should I collect this again when the handbook changes?
Yes, if you want a record for the new version. Put the version or date on the form so you can tell the records apart.
Can I pre-fill the employee's name and start date?
Yes. Import a spreadsheet and send each person a personal link. You can lock the fields you already know.
Where does the signature go?
On the form itself. FormSign also builds a letter from the answers, with a signature line, when you add a document to the workflow.
Do I need an account to try it?
No. Open the template, edit the acknowledgment text and test it. You'll create an account when you want to save it and send it.
