Independent Contractor Onboarding Form

What is an independent contractor onboarding form?
An independent contractor onboarding form is the practical questionnaire you send before the work starts. It covers contact details, the services, dates and how you will describe the fee. Official tax forms stay separate.
When to use it
- As soon as you agree to work with a freelancer or contractor.
- When several contractors start the same project and you want the same questions.
- When you want one board row per contractor instead of a thread of emails.
- When the kickoff also needs a signature on your own contractor letter.

Why FormSign
Each contractor becomes a row on your board. Import a spreadsheet if you already have names and start dates, and lock those fields. Add your own contractor letter to the workflow and FormSign fills its placeholders from these answers, then sends it for e-signature.
This template is a starting point, not legal advice. Worker classification and contractor paperwork vary; check what applies to your business.
Questions
Does this replace a W-9 or a contractor agreement?
No. Use your official tax form and your own agreement for those. This form is the practical onboarding questionnaire.
Why doesn't it ask for a Social Security number, EIN or bank details?
Those belong in your tax and payment process, not a general onboarding form. This template leaves them out on purpose.
Can I pre-fill the contractor's name and start date?
Yes. Import a spreadsheet and send a personal link with those fields already filled in.
Can they sign online?
Yes. The form ends with a signature and a date. You can also add a separate letter for e-signature.
Do I need an account to try it?
No. Open the template, edit it and test it. You'll create an account when you want to save it and send it.
