Payroll Change Request Form
A pay or job change is easier to approve when the new value and the reason are written down. This payroll change request form records the employee, the effective date, the kind of change, the new value, the reason, and an approver's name and signature. The kinds of change are pay rate, title, hours, department, and address. It does not collect a payment account, and it does not name or recreate a government form. Edit the list if you need to, try it, then send one link for each request.

What is a payroll change request form?
A payroll change request form is the record of a requested change: who it is for, when it starts, what is changing, and who approved it. Updating pay stays in your usual payroll process.
When to use it
- When a pay rate, title, or weekly hours is about to change.
- When someone moves to another department.
- When a home or mailing address on the employee record needs an update.
- When the approver should sign the request.

Why FormSign
Each request becomes a row on your FormSign board. The starter letter uses each question's label as a placeholder, so the approval note matches the form.

Questions
Which changes can someone request?
Pay rate, title, hours, department, or address. Edit the dropdown if your list is shorter.
Should they enter a payment account here?
No. This form has nowhere for that. Keep payment setup in your usual payroll process.
Does submitting this update pay by itself?
No. It records the request and the approval. Apply the change in the payroll process you already use.
Can I remove address from the list?
Yes. Delete that option if you collect address changes somewhere else.
Do I need an account to try it?
No. Open the template, edit the change types and test it. You'll create an account when you want to save it and send it.
