Receipt Submission Form
A receipt is easier to review when the amount, the vendor, and the file arrive together. This receipt submission form collects the business, the purchase date, the vendor, the amount, how it was paid, what it was for, a category, the receipt, and notes. Payment choices are card, cash, check, or other. It does not ask for a card number or a payment account. Edit the categories, try it, then send one link per purchase.

What is a receipt submission form?
A receipt submission form is the record of one purchase: who it was for, the vendor, the amount, and the receipt file. How you treat the purchase stays in your usual process.
When to use it
- When a client or employee paid for something and needs the receipt on file.
- When several receipts arrive in the same week.
- When you want the vendor and the amount next to the file.
- When a category helps you sort the board later.
What to include
Who it is for

What to leave out
Do not ask for a card number or a payment account. This form does not say how the purchase should be treated.
Why FormSign
Each receipt becomes a row on your FormSign board, with the file on that row. The starter letter uses each question's label as a placeholder, so a copy of the submission matches the form.
This template is a starting point, not legal advice. It records a purchase and a receipt. It does not say how the purchase should be treated.
