Tax Year Document Checklist

Chasing tax documents by email is slow, and clients should not type account numbers into a form. This tax year document checklist asks them to upload W-2s, 1099s and the other paperwork you name, then sign that they have sent what they have. Edit the list for your practice and send one link per client.

What is a tax year document checklist?

A tax year document checklist is the request you send a client for the files you need to prepare the return. Each item is an upload, so the papers arrive with the client's name and year already attached.

When to use it

  • At the start of tax season, one link per client.
  • When a client still owes a W-2, a 1099 or last year's return.
  • When you want uploads on the board instead of in a shared inbox.
  • Beside your organizer or engagement letter, in the same workflow.

What to include

Who and which year

Client name, email, phone and the tax year.

Uploads, not typed numbers

W-2s, 1099s, last year's return and the other files you need.

A place for leftovers

One more upload and a note for anything that does not fit a slot.

A signature and date

They confirm they uploaded what they have and will send the rest later.

What to leave out

Do not ask clients to type Social Security numbers, EINs, bank details, routing numbers or account numbers.

Why FormSign

Each client is a row. You can see who has uploaded and who has not started. The starter letter lists every checklist label as a placeholder, so a follow-up document matches the form. Add your engagement letter to the same workflow if you want that signed too.

This template is a starting point, not legal or tax advice. Adjust the list to the returns you prepare.

Questions

Should clients type account numbers on this form?

No. Ask them to upload the statement. This template has no field for account numbers, routing numbers or tax IDs.

What if they don't have every document yet?

They can upload what they have and use the note to say what's missing. The signature says they'll send the rest later.

Can I change the upload list?

Yes. Add or remove upload questions so the checklist matches the returns you prepare.

Can I send this with the organizer?

Yes. Use this checklist for the files, and the tax organizer for the questions. Both can live in the same workspace.

Do I need an account to try it?

No. Open the template, edit the list and test an upload. You'll create an account when you want to save it and send it.
