Training Completion Acknowledgment
A finished training is easier to find later when the name, the date, and a signature are in one row. This training completion acknowledgment asks for the employee, the training name, the date completed, the trainer or provider, how it was delivered, a confirmation, an optional certificate, and a signature. Edit the delivery list, try it, then send one link when the session ends.

Why FormSign
Each completion becomes a row on your FormSign board, with the certificate on that row when they attach one. The starter letter uses each question's label as a placeholder, so a copy of the record matches the form.
This template is a starting point, not legal advice. It records that a training was completed.
Questions
Does signing this prove anything beyond the record?
No. It records that they completed the named training. Keep certificates and other records in your usual process.
Is the certificate required?
No. The upload is optional. They can sign without attaching a file.
What belongs in delivery?
In person, live online, recorded, or other. Edit the dropdown so it matches how you actually train.
Can I use this for more than one course?
Yes. The training name is a text field, so each completion can name a different course.
Do I need an account to try it?
No. Open the template, edit the delivery list and test it. You'll create an account when you want to save it and send it.
