Performance Review Form
A review is easier to remember when the ratings and the comments are in one place. This performance review form records the employee, the reviewer, the period, a goal rating, an overall score, strengths, areas to grow, the employee's comments, and both signatures. It is a conversation record. It makes no employment-law claims. Edit the wording, try it, then send one link for each review.

What is a performance review form?
A performance review form is the written record of a review: who was reviewed, who reviewed them, the period, the ratings, and the comments. It does not make an employment decision.
When to use it
- When a review period is ending and you want the same questions each time.
- When the employee and the manager should both sign the notes.
- When several reviews happen in the same month.
- When comments should live on a board, not in a slide deck.

The period and the ratings
A period date, a goal rating, and an overall score.

Why FormSign
Each review becomes a row on your FormSign board. The starter letter uses each question's label as a placeholder, so the review note matches the form.
This template is a starting point, not legal advice. It is a record of a review conversation, not an employment decision.
Questions

No. It records the conversation. Decisions about role or pay stay in your usual process.
Does it make employment-law claims?

What do the scores mean?

Can the employee add comments after the meeting?
Yes. Employee comments are on the form. They can fill them in before they sign.
Do I need an account to try it?
